Wings for Children, Flight Division, a non-commercial, non-profit, volunteer public service organization and it’s volunteer pilot(s)

hereby agree to provide the following passenger(s)

with air transportation, free of charge, for the passsenger(s) convenience, assisting with or returning from a situation in foster care, child abuse/neglect, abandonment or missing; and/or medical treatment or diagnosis.

In consideration for receiving this air transportation, we agree to hold harmless Wings for Children, Flight Division. To assume all risk of damage or injury, including death, however caused, whether by negligence of the pilot(s), owner(s) or otherwise, while in or about the plane, and agree that accomplishment of the objective of flight is not guaranteed; neither they nor their hiers will make any claim against or sue Wings for Children, Flight Division, pilot(s) or plane owner(s) from all claims that they or their heirs may have for any of the foregoing, however caused, whether by negligence of the pilot(s) or owner(s) or otherwise, and waive all rights, claims and causes of action against Wings for Children, Flight Division, pilot(s) and plane owner(s). To hold harmless and indemnify Wings for Children, Flight Division, pilot(s) and plane owner(s) from all loss, liability, damage or costs incurred because of any claim made or lawsuit brought relating to the intended transportation, wheterh based on neglicgence of the pilot(s) or owner(s) or otherwise.

The above agreement toWings for Children, Flight Division, includes its members, directors, officers, employees and volunteers.

In the event any portion of this contract is held invalid, the other sections shall remain in full force and effect.

This waiver is governed by the laws of South Carolina.

As evidenced by my signature below, I have read this document entirely and agree to its terms.

PASSENGERS AND/OR CARGO OWNER CONFIRM THAT THEY HAVE CAREFULLY READ AND UNDERSTAND THIS AGREEMENT AND THAT IT IS A RELEASE OF LIABILITY FOR NEGLIGENCE OF THE PILOT(S) OR OWNER(S) OR OTHERWISE AND ARE SIGNING OF THEIR OWN FREE WILL.

Photo Release: I understand tha tin order to continue community service, Wings for Children, Flight Division relies upon contributions which are in part solicited through publicity. In order to assist with these efforts, I grant Wings for Children, Flight Division, permission to take and use my photograph for promotional and public relations.
